    Job Summary


    Internal Medicine of Brighton is an established private internal medicine practice in Rochester, NY located near MCC providing exceptional high quality primary care to adult patients (18 yrs +). We are looking for an APP to expand our practice and join our medical family.

    The ongoing model includes working collaboratively with the physician, a care manager and a pharmacist.

    Responsibilities

    • Responsibilities include evaluation and management of acute/chronic illnesses, ordering/interpreting/communicating diagnostic tests and results, hospital/ED follow-ups and routine well visits, transition of care management, collaboration with physicians and medical team regarding patient care, facilitating appropriate referrals to specialists or community-based agencies. Opportunities in patient education/counseling intended to maintain or restore physical/emotional health and function, and educational support for various learners consistent with our goals.

    Excellent communication and collaboration skills are essential with the ability to exercise independent judgement and initiative.

    The candidate must have completed NYS NP or PA certification along with licensure and have applied for DEA registration prior to hire/start date.

     

    Schedule: 4 Day Work Week


    Monday, Tuesday, Thursday, Friday (OFF Wednesday)

    8:00 a.m. to 4:00 p.m.

    On-Call: 1 in 6 and there will bel an additional stipend paid when on-call.

     

    Qualifications

     

    • Current Adult Nurse Practitioner of Physician Assistant certification with a valid state license.

    • Patient Satisfaction. Interacts on a frequent, regular and daily basis with patients, family members, medical/social/public services support, regulatory agencies and/or payor agencies.  
    • Medication & Orders. Prescribe medication, write orders, perform procedures and respond to emergency and/or immediate condition changes of acute patients.
    • Documentation. Obtain complete, detailed and accurate patient histories, review patient records, perform physical exams, record all pertinent data to develop comprehensive medical status, diagnosis and clinical treatment plan for all assigned patients.
