Physician House Calls-Staff Physician Part Time 24 hours/week (3 days/week)
Salary Range: $138,000 to $156,000 (Pay information will vary depending upon relevant experience for the position; this is for 60% time) plus variable compensation based on performance.

GENERAL SCOPE: The physician provides primary care including direction and coordination of medical care for patients in the Physician House Calls practice. Physician House Calls is a unique primary care home visit program serving homebound, primarily older, persons in the eastern portion of Monroe County. This program provides high quality primary medical care using an interdisciplinary approach including an advanced practice provider, social worker, and registered nurse-coordinator. Program objectives include maximizing quality of life for participants and emergency department and hospital avoidance. The physician will be responsible for performing home visits, attending weekly team meetings, and collaboration with the rest of the interdisciplinary team.

 

EDUCATION / QUALIFICATION:

  • Doctor of allopathic or osteopathic medicine.
  • Licensed to practice medicine in New York State.
  • Board Certified in family medicine or internal medicine required. Additional board certifications in geriatrics and/or hospice and palliative medicine desirable but not required.
  • Unrestricted, valid driver’s license.

ESSENTIAL FUNCTIONS:

  • Provide medical care that meets standards of practice, the needs of patients and families, regulatory requirements, and documents such services.
  • Perform appropriate home and telehealth medical visits.
  • Coordinate care with interdisciplinary team and, where appropriate, with facility staff and other appropriate service providers and medical providers.
  • Skilled in use of electronic medical record.
  • Participate equitably in the on-call coverage system as assigned.
  • All other job duties as assigned.

EXPERIENCE:

  • At least 3 years of primary care experience preferred.
